Please use this form to request assistance with an incoming or outgoing NDA/CDA or MTA/DTA associated with an active or forthcoming sponsored project. Miriam will be out of the office starting Friday, December 20th and returning Monday, December 30th. Upon return, Miriam will review submission requests in the order they were received.

Note: If you are in the early stages and are seeking an NDA for preliminary discussions with a collaborator, and are unable to complete the webform because not enough information is known, please contact the Technology Licensing Office. Any executed agreements should be uploaded into eProposal, when/if a DSS is created.
