FORMS-G Application Forms
Applicants must use FORMS-G application packages for NIH and other HHS agency due dates on or after January 25, 2022, and must use FORMS-F application packages for due dates on or before January 24, 2022.
Overview of the FORMS-G changes:
- Most of the changes are updating the various forms’ OMB expiration dates (actually not visible in Cayuse) and replacing the “DUNS” field label with “UEI” (Unique Entity Identifier) label
- Minor changes to the PHS Fellowship Supplemental Form, PHS Human Subjects and Clinical Trials Information Form, and SBIR/STTR form
- NIH is expanding the R&R Budget forms Section F Other Direct Costs free text lines from 3 to 10 (so the 3 free text lines F8, F9, and F10 are expanding to 10 free text lines)
NIH is adding the FORMS-G package to all FOAs, so in Cayuse, you will need to choose between the FORMS-F package for applications submitted for due dates before Jan 25, and FORMS-G for applications submitted for due dates on or after Jan 25 (start of Cycle I). If you do not seen a FORMS-G option, please redownload the opportunity into Cayuse. In previous years, NIH issued new parent opportunities for the new forms packages, but with FORMS-G they are just attaching the FORMS-G package to all open opportunities.
